Hard copies of the application and all the required documents should be sent (or handover) to;<\/p>\n<p><strong>Dr. Thakshila Adikari,<\/strong><\/p>\n<p><strong>The Secretary,<\/strong><br \/><strong>Ethics Review Committee,<\/strong><br \/><strong>Faculty of Livestock, Fisheries and Nutrition,<\/strong><br \/><strong>Wayamba University of Sri Lanka,<\/strong><br \/><strong>Makandura, Gonawila (NWP)<\/strong><\/p>\n<p>When mailing the applications it is required to submit both a <strong>true copy<\/strong> and a <strong>blind copy<\/strong> of all application documents for Ethics Review.<\/p>\n<ul>\n<li>The true copy should include the names and details of all investigators.<\/li>\n<li>The blind copy must have all investigator names and identifying details removed.<\/li>\n<\/ul>\n<p>Please submit these as <strong>two separate email submissions<\/strong>:<\/p>\n<ol>\n<li>One email containing the true copy<\/li>\n<li>A second email containing the blind copy<\/li>\n<\/ol>\n<p>Both submissions should clearly indicate the study title in the subject line.<strong><\/strong><\/p>\n<p>An application for the review of the ethics of proposed biomedical research should be submitted by a qualified researcher responsible for the ethical and scientific conduct of the research.<\/p>\n<p>All documentation required for a thorough and complete review of the ethics of proposed research should be submitted by the applicant.<br \/><strong>The complete application consists of all the following:<\/strong><\/p>\n<ul>\n<li>Duly completed application form (which can be downloaded from this website at Downloads Page)<\/li>\n<li>Information sheets, consent forms, assent forms and questionnaire forms, where necessary in all three languages (Sinhala, Tamil and English).
